Contact your GP surgery online or by phone if you think you are due to have Long Term Condition Review but have not been sent an invitation, You should hear from your GP Surgery around your birthday month.
It’s important to go to your appointment unless you have symptoms of COVID-19. All NHS services are making sure it’s safe for you to attend.
What is a LTC(Long Term Condition) Review
A long term condition review is a type of Health Check for people with Long Term Conditions (eg diabetes, asthma & stroke)
Health checks for people with long-term conditions, eg diabetes, asthma & stroke
If you have one of the following conditions then you will be invited for an annual review or more frequently if necessary.
- – Diabetes
- – High blood pressure
- – Kidney disease
- – Heart disease
- – Asthma
- – Chronic bronchitis
- – Rheumatoid arthritis
- – Learning disabilities
- – Dementia
- – Severe mental health problems
We will contact you shortly before your review is due. You may need a blood test before the appointment, in which case the invite will say this.
The appointment will be 30 minutes long, however if you have more than one problem it may be longer.
