To manage and coordinate all aspects of practice functionality, motivating and managing staff, optimising efficiency, ensuring the practice achieves its long-term strategic objectives in a safe and effective working environment. Through innovative ways of working, lead the team in promoting ED&I, SHEF, Quality & CI, Confidentiality, Collaborative Working, Service Delivery, Learning and Development and ensure the practice complies with CQC regulations.

Doctors

Work as part of the wider team delivering high quality medical care and screening to an area of high need. Work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the PMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

Advanced Nurse Practitioner

To work as an autonomous practitioner, responsible for the delivery of Minor illness and delivery of Triage services.  They will also work closely with the Lead Practice Nurse to manage & improve Chronic Disease Monitoring across both sites and to create, develop and deliver a minor surgical service for our patients.

Practice Nurses

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Health Care Assistants

To support the practice nursing team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The healthcare assistant will deliver  and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education.

Administration Teams

Administrators

To be responsible for undertaking a wide range of secretarial and administrative duties and the provision of administrative support to the multidisciplinary team. Duties can include but are not limited to, the processing of information (electronic and hard copy) in a timely manner, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers and in accordance with current policies, including the use of the electronic referral service (ERS).
