New Patient Registration

To register at any of our Practices, complete a registration form obtained from our reception staff or digitally. Proof of residence is essential (recent utility bill, bank statement).

You will also be given a new ‘Patient Health Questionnaire’ to complete. If you have a child you must complete our new ‘Children’s Health Questionnaire’.

Your medical records often take a considerable time to arrive from your previous doctor and this gives us the opportunity of recording some basic information about you and offering you any immediate care you may need. We appreciate your patience whilst moving over.

Named Accountable GP

NHS Employers and the General Practitioners Committee of the British Medical Association have agreed that all patients will have a named accountable GP

The ‘named’ GP will:

  • * Take lead responsibility for ensuring that all appropriate services required under the contract with the * practice are delivered to you.
  • * Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver multi-disciplinary care package that meets your needs.
  • * Ensure that your physical and psychological needs are recognized and responded to by the relevant clinicians in the Practice.

The Villages Medical Group– will ensure that there is a named accountable GP assigned to each patient.

New patients will be allocated a GP at the time of registration.

Your named accountable GP may be the same as your registered GP however this does not affect your ability to see any GP of your choice as you currently do.

Our GPs are registered with the General Medical Council.

Change In Personal Details

Please inform the reception staff as soon as possible if you change your name, address, marital status or telephone number, so we can keep our records accurate.

If you move out of area, we ask you contact our receptionist team who will discuss with the Lead GP weather you’re able to stay at our practice or weather you’d be required to move to a different GP Practice
